2009 Classic Spring Training Sign Up Form
To sign up, please fill out the following information as best you can and send your $100 payment to Curt Hollis/NFHC at 2974
Hartley Road West, Jacksonville, Florida 32257. Payment can be check or money order. Paypal users send an on-line payment
to curt@classicspringtraining.com. Please add $5 to cover the Paypal fees. Your name will be added to the attendee list as soon
as the payment is received.
Refund Policy: Refunds in full will be made until January 1, 2009. No refunds after January 1,
2009.

In the past, we have had some people sign up
for the event and tell me they will bring their
payment, then not show up. This leaves me
stuck with the extra cost for their sweatshirt,
banquet dinner and their portion of the
insurance, fields, light cost, etc. I must ask that
you please submit your payment when you
sign up. Hardship cases must work directly
through me. Your name will be added to the
attendee list when your payment is received.

Is your spouse attending? Will you be bringing any other people such as you son or daughter? As a attendee your participation in the event,
your banquet ticket including dinner and an embroidered sweatshirt are included in the total price.  Please read the following questions and
answer accordingly so we don't miss anyone.
Including yourself, enter the total number people that you will bring to the Saturday evening
dinner/banquet. As a attendee, your meal is paid. Additional people, such as your spouse, would be $15 per
person.
As a paid attendee, you will receive 1 embroidered sweatshirt. Please list your size in the box. If you need any
additional sweatshirts, the cost is $17 each, Please list the number of any additional sweatshirts needed and
their sizes. Example XXL, 1 additional, xl.
